We help ensure safe and smooth transitions to home from health care facilities and provide in-home comprehensive treatment, Living assistance, support & education. Our focus is on improving outcomes of in-home care for our patient’s to reduce unplanned re-hospitalizations, and to reduce cost of care. We help you rehabilitate from your care needs like a stroke or surgery, or managing chronic conditions such as diabetes, heart disease, and arthritis, or other conditions requiring supervision or assistance with your daily life. Private Duty Nursing is provided when you need long term care or may just wish to supplement your existing entitlement. Patient Care & Safety is Important to Us!
COVID-19 Support
As we fight through this unprecedented time of the COVID-19 pandemic, our number one priority during this fearful situation is the health and safety of our patients and employees. We are maintaining our ability to provide our patients a long-term care by continuously monitoring and swiftly responding to the mutable conditions.
Our EMR system developers have added patient pre-screening module that makes our caregivers to complete first at the start of each visit clock-in (at patients’ home) before she can start other shift tasks; our caregivers and office staff are also pre-screened for COVID-19 regularly, and we will continue to pay our hourly staff for covering their scheduled shifts. Furthermore, we offer extended leave for our employees if needed to fully recover from the potential sickness. With our full hearts and thoughts, we will continue to provide services to our patients at the comfort of their home and at the same time will continue to keep our employees motivated.
